Every person needs the support of others, but we look for very special people to support elders, including those with complex medical or rehabilitative needs. As an Intake Coordinator with ADH you'll be one of those special people.

Adult Day Health is seeking an Intake Coordinator to manage and accept referrals. This individual will initiate appropriate communication with clinical and operations teams based on the needs of referred clients/consumers to facilitate appropriate placement and/or services.

Qualifications:

Requirements:

* Bachelor's degree in human services field or equivalent in education and experience combined
* Two years of related experience
* Haitian Creole Speaking Preferred
